This bit of circuit trickery is officially called "Bootstrapping". This was used by MCI in their 500 series mixing consoles; in order to increase the headroom & voltage swing on certain amplifiers. I had worked at the factory in Ft. Lauderdale, Fla., in the early 70's. I was hired as service manager at their dealer in Nashville in mid 76.
I have attached a portion of the 500 Consoles Input Module schematic; the circuit in the circle is a complete opamp w bootstrapping. I also found an article on bootstrapping IC's, by two Applications Engineers with Analog Devices. I envision applying this technique to an IC, driving a pair of output transistors for extra drive, from the amp.
