Those triple flanges that the ER's come with are the only fit I can't get to work in my ears!! I have some noise reducers that I sometimes wear that have the same thing, but boy, do they hurt after a while. I get a choice - deafness or pain. Then I fit the Senn customs and just tuck the lead into a pocket!!

Actually Ian you and others have mentioned the lack of comfort. Personally I felt it was true for about 2 weeks until my ears got accustomed and now they are my favorite type. However for those who find the tri-flange uncomfortable after two weeks I would suggest trying the foam tips by Comply. The foam tips work wonderfully, isolate better, increase the bass by virtue of a better seal, can be washed periodically to extend their life and are super comfortable. The downside is cost.
I almost forgot you can have ear molds made to fit the Etymotic IEM. Good luck!
